REPEAT PRESCRIPTIONS

We request that patients either email/ post / fax their prescription request form with their name, address and date of birth clearly written or their pharmacy list of medication (this can be requested at your local pharmacy). This list will be returned to the patient after each prescription is issued.

Alternatively, you can drop your prescription request form in the post box located at the surgery entrance.

To ensure accuracy and safety please note that repeat prescriptions cannot be ordered over the phone.

The turnaround for repeat medication is a minimum of 48 hours.

We encourage you to download our prescription request form and fill it in completely and return it to us.

An incomplete prescription request form will result in a delay in your prescription.
